Unexpected Delayed Ventilator Weaning after General Anesthesia in a Patient with Achondroplasia - A Case Report -

Abstract
Achondroplasia is the most common non-lethal skeletal dysplasia, and results in dwarfism. An understanding of the multiple abnormalities, such as disproportionate dwarfism, midfacial hypoplasia, deformities of the spine, foramen magnum stenosis, that affect achondroplasia will facilitate the safe delivery of anesthesia in these patients. We describe the perioperative management of a 33-year-old man with achondroplasia who underwent general anesthesia for a chronic otitis media surgery. Endotracheal intubation was not difficult with a 5.5-mm cuffed tube. Emergence period was complicated by high airway pressure and desaturation probably caused by mechanical obstruction following position change. The postoperative course was complicated by respiratory compromise requiring prolonged ventilator care.
